Nurses (RN)

Hospice doesn’t directly administer medical care, but we assist in obtaining and setting up necessary medical equipment. We also supply essential medical items like bed pads and gloves, sparing patients and families the need to source them separately.

CNA

Facing a serious illness can stir up emotional challenges. Lifted Hospice embraces the complete well-being of our patients, offering therapy to address emotional concerns surrounding terminal diagnoses and end-of-life issues. We also have a dedicated clergy member for spiritual support.

Social Worker

Our hospice team supports family caregivers by providing insights into their loved one’s illness. We offer guidance and education regarding the care of specific disease symptoms.

Patients dealing with a serious illness may find daily activities challenging. Hospice aims to help with day-to-day care, including tasks like cooking, cleaning, personal grooming, restroom use, bathing, and home dental care.<span style=”background-color: var( –e-global-color-primary ); color: var( –e-global-color-accent ); letter-spacing: -0.2px;”></span>

Chaplain

We extend our services during temporary hospital stays to ensure patients receive the care they need.

Medical Director / Doctor

In respite care, our team temporarily assumes the caregiver’s responsibilities to give them a much-needed break.

Hospice Liaison

A terminal diagnosis often triggers strong emotions in patients and their families. Lifted Hospice offers specialized services to help ease the pain of loss, including grief counseling. These services remain available to the patient’s family for up to 13 months after the patient’s passing.

Nurses (RN)

Our Registered Nurse (RN) Case Managers coordinate each patient’s personalized care plan, managing symptom relief and providing regular visits and on-call support to ensure compassionate and attentive medical care.

CNA

CNAs assist patients with daily living activities, such as bathing, dressing, and grooming, enhancing comfort and dignity through consistent, compassionate personal care.

Social Worker

LVNs work closely with RN Case Managers, providing hands-on nursing care, routine medical support, and medication assistance as part of each patient’s care plan.

Social Worker

Our Social Workers offer emotional and practical support to patients and families, helping with care planning, resources, and navigating the complexities of hospice care.

Chaplain

Chaplains provide spiritual support tailored to each patient’s beliefs and values, offering counseling, prayer, and comfort during challenging times.

Medical Director / Doctor

Hospice physicians and medical directors oversee patient care plans, making necessary adjustments to ensure medical needs are met in alignment with each patient’s wishes.

Hospice Liaison

Our Hospice Liaison is a dedicated point of contact, answering questions about hospice care, providing guidance to patients and families, and ensuring a smooth transition into hospice services.

Admin Team

The office team coordinates scheduling, handles paperwork and insurance questions, and provides logistical support, creating a seamless experience for families and patients.
