When your medical team and family decide that hospice care is the best option for you, we'll be with you every step of the way. Interestingly, some patients even start to feel better in certain ways once they begin hospice.
Getting started with Lifted:
Patients can come to us in two ways. Either a doctor refers them to us, and in that case, we work closely with the doctor to review all the medical records and assess the patient's condition. Alternatively, families can contact us directly, and we will coordinate with the patient's physician to start the referral and medical review process.
A Lifted Hospice Community Liaison will get in touch with you to review your benefits and complete any necessary paperwork. Once the paperwork is done, a nurse will visit the patient for a thorough evaluation, which helps create a personalized care plan.
If the patient meets the eligibility criteria, they will be admitted to our care, and a tailored care plan will be put into action. This plan includes a team of professionals and visits that cater to the patient's unique needs. Families can expect updates from their dedicated RN Case Manager after each visit.
