Lifted Hospice is Medicare-certified, and currently accepts Medicare only. We are actively working through the process to begin accepting Medicaid and private insurance. For Medicare beneficiaries, all hospice-related services — including medications for your terminal illness, durable medical equipment (DME), and nursing visits — are fully covered.
Lifted is a newer hospice, but we’ve grown quickly by holding fast to one principle: serve people the way we’d want to be served. Our leadership brings decades of experience in healthcare, pharmacy, and home health.
That’s not only normal, it’s expected. We provide pre-death grief support as part of our spiritual and emotional care.
Yes. We believe medical equipment should never be an afterthought. We provide all necessary durable medical equipment (DME) — like hospital beds, oxygen, walkers, and more — to ensure safety, comfort, and dignity at home.
You’ll be supported by a consistent care team, including a registered nurse (RN), social worker, hospice aide, chaplain, and physician. We assign sustainable caseloads, not cookie-cutter ratios, so our team can give you the time and attention you deserve.
We don’t use a fixed ratio. Each nurse has a balanced, sustainable caseload that may vary depending on geography, acuity, and family needs — always with quality as the top priority.
You’ll speak with a real person immediately. A nurse will assess the situation and, if needed, be dispatched without delay. Your safety and comfort guide our urgency.
Our physician and registered nurse (RN) collaborate with you and your family to create a personalized plan of care. We proactively manage pain, breathing changes, anxiety, nausea, and other symptoms with medications, repositioning, emotional support, and more — all centered around comfort.
We review all your medications. If they support your comfort or symptom control, they may be continued. Medications unrelated to your goals may be adjusted in conversation with your doctor.
Yes. If needed, we’ll coordinate short-term inpatient care for symptom control in a participating hospice facility or contracted nursing facility — with the goal of returning you home as soon as possible.
We prioritize clear, ongoing communication with both patients and families. You’ll always know what’s happening — and what to expect — through consistent contact and updates from your care team.
Absolutely. Your values and preferences are central. We make decisions with you, never for you.
Call us any time — 24/7. You’ll reach a compassionate staff member who listens and takes action. We respond quickly to ensure your concerns are addressed with care.
Yes. If your attending physician wishes to remain involved, we will coordinate directly with them. You can choose whether to receive care from our medical director or keep your current doctor involved.
We offer gentle, honest education and emotional support at every stage. From early signs of decline to the final days, we walk with you — ensuring no one is caught off guard.

We offer respite care, trained volunteers, social work support, grief counseling, and chaplain visits. If you need rest, we can arrange short-term inpatient respite care, giving you a needed break while your loved one receives continued support.
We will support you in transitioning care to a skilled nursing facility or other contracted setting, if needed. You’re never alone in this process.
We offer bereavement services for up to 13 months after the loss of a loved one. Support includes check-ins, grief counseling, and resources for healing. If it’s been longer than 13 months and you’re still grieving — please call us. We’re still here for you.
