Social Worker

Hospice care is specialized support for individuals facing a life-limiting illness. Rather than focusing on curative treatment, hospice prioritizes comfort, quality of life, and emotional and spiritual well-being. It is a holistic approach that supports both the patient and their loved ones during the final stages of life.

Patients who have been diagnosed with a terminal illness and a life expectancy of six months or less (as certified by a physician) are typically eligible for hospice. However, hospice care may continue beyond six months if needed. Eligibility also includes a desire to shift the focus from aggressive treatment to comfort care.

If curative treatment is no longer working or your loved one’s focus has shifted to comfort and quality of life, hospice may be appropriate. Common signs include frequent hospital visits, rapid decline, or increasing need for help with daily activities. Our team can assist in evaluating whether hospice is the right step.

Absolutely. We offer comprehensive support services including emotional counseling, education, respite care, and grief and bereavement support. We consider the entire family part of the care plan.

Yes. Your primary physician can remain involved and collaborate with our hospice team to coordinate your care. You do not have to give up your current doctor to receive hospice services.
